I'm really confused about adding a chapter character

The title pretty much speaks for itself. I have already submitted a comment to a chapter that's missing a character, but only after that did I look at the help tab. And that's where I got lost. So, to try and keep this simple, I just have two questions about the system.
If you notice that there is a character that has not been added to closed chapter, do you just comment with the missing character's name (at the chapter overview page), or do you have to fill in a special form? I'm just asking because I didn't see a form, but I wouldn't put it past myself to have overlooked it.
Steps number two & three are pretty much just for chapters that are awaiting approval - correct?
I know the answers to my questions are pretty simple, but I'm just not getting it right now.

But you just reminded me that I must have broken something recently, because I have stopped getting email notifications when people leave comments on those pages.
So for this chapter could you just tell me here or on the chapter itself? And I will reopen the chapter if it's necessary, so we can add the character.

As for the other chapter, the "rules" have been kind of ambiguous about characters who aren't really in the chapter. This is especially true for those earlier chapters, where a chapter is not necessarily a nice, tidy, self-contained "scene," but instead a rambly chunk of recent Lotherian history.
However, for that chapter I would say those characters deserve to be in it. They are all part of the "scene" of Alred walking up to Githa's house. So MOAR KARMAZ for you!
